How to Make a Referral

We welcome referrals from local authorities, social workers, health professionals and other commissioners for children and young people aged 0–18.

Initial enquiry - Contact our team to discuss the child or young person’s needs.
Submit Referral Form – Complete and send our referral form, along with relevant reports or assessments.
Assessment – We review the referral and carry out a compatibility and needs assessment.
Visit & Planning – Families and professionals are invited to visit the home. Transition planning begins.
Placement – A safe, well-matched placement is agreed, and the child moves in with support for a smooth transition.
